What information do we collect?
We collect information from every prospective patient when they fill out a form or inquire in person, by phone or on-line about vas reversals or sperm retrieval. This allows us to stay in contact before, during or after the vas reversal to provide continued care. When contacting ICVR, Arizona Andrology Lab and Cryobank (AALC) or registering on our site, in person or any format, as appropriate, prospective patients may be asked to provide their name, e-mail address and/or phone number or other information so that we can continue to provide care.

How do we protect personal information?
We implement a variety of security and cyber measures to maintain the safety of everyone’s personal information when they enter, submit, or we access personal information. We regularly review and update all security features, and review cyber security policies and alerts with all staff.
We use a secure server. All supplied sensitive/credit information is transmitted via Secure Socket Layer (SSL) technology and then encrypted into our Database to be only accessed in office by those authorized with special access rights to our systems, and are required to keep the information strictly confidential at all times and are NOT to be shared with anyone.
After a transaction, all critical private information (credit card numbers, social security numbers, financial information, etc.) is not stored on our servers. At the completion of any financial relationship when all bills are paid in full, all financial, credit or payment information is removed from the records and securely eliminated and destroyed.
Do we use cookies?
We do not use cookies. (Dawn, our Senior Patient Care Coordinator, likes to point out that she enjoys eating cookies).
Do we disclose any information to outside parties?
We do not sell, trade, or otherwise transfer or provide to outside parties any of our patient’s personally identifiable information. This does not include trusted third parties who assist us in operating all aspects of our website, social media, search engine optimization, live chat, conducting our business, or servicing and caring for our patients, so long as those parties agree beforehand to keep this information strictly confidential and not share this with anyone. We may also release any information when we believe the release is appropriate to comply with any federal, state, or local municipal laws, enforce our website policies, or protect our staff, vendors and patients or others rights, property, or safety.
Non-personally identifiable patient information, where there is no way to trace this information to each individual, may be provided to other parties for promotion, marketing, advertising, research, publication, presentations or for other appropriate and controlled uses.
